School Account Renewal Form

SCHOOL CHOICE© programming is available at no charge to accredited public and private schools grades K-12. Each year, school accounts are required to renew enrollment in the DIRECTV GOES TO SCHOOL program in order to continue receiving complimentary programming.

Use this form to update your school’s contact information and verify all the receivers on your account.

School Information

DIRECTV requires two contacts for each school account. Please provide your contact information below, in addition to contact information for one other faculty member:

Receiver Information

Please provide the Access Card Number and/or Receiver ID for each of the receivers on your school’s account. Receivers that are not renewed will be disconnected. Tip: Store your receiver information to save time during next year's renewal process.
