DIRECTV® Goes to School - Sign Up & Get Started

Bringing DIRECTV to your school is easy

Here's What You Need to Do

	Read and sign the DIRECTV Commercial Viewing Agreement and the Semi-Public Addendum.
	Complete the Program Authorization Form.
	Write a letter on your school letterhead requesting the SCHOOL CHOICE® package. Also indicate how many receivers your school will need. You can request up to 4 standard receivers - or up to 3 standard receivers plus 1 DIRECTV Plus^® DVR. To record programs, you'll need the DIRECTV Plus^® DVR receiver.
	Fax all 4 completed documents (6 pages in total) to 1-303-712-4995 or scan and e-mail them to schools@directv.com.

Your FREE equipment will be shipped to your school once all your application materials are received and approved. You will need to arrange installation. (DIRECTV will not be able to provide this service.) You can find professional installers in your local listings.

After the installation is complete, call 1-888-330-7827 to activate your receiver(s). All equipment must be installed and activated within 90 days of receipt. Have the manufacturer name, serial number, receiver identification number and the access card number for each receiver handy when you call. This information can be found on the box, the access card sticker - or on the screen once your installation is complete.
